ABSTRACT

Objective:To explore the effect of changes in BMI (ΔBMI) on left ventricular function in early breast cancer patients undergoing anthracycline chemotherapy.Methods:The clinical data of 170 breast cancer patients treated in the Lianyungang Oriental Hospital from January 2018 to October 2021 were retrospectively analyzed. The clinicopathological data and cardiac color doppler ultrasound examination results of the patients were collected. Single-factor and multiple-factor were used to analyze the risk factors of cardiotoxicity in patients after chemotherapy. ΔBMI was calculated and the receiver operating characteristic (ROC) curve was drawn; the cut-off value of the ΔBMI was measured to obtain the diagnostic accuracy.Results:Compared with before chemotherapy, the mean values of BMI, left ventricular end-diastolic volume (EDV), left ventricular end-systolic volume (ESV), left ventricular end-diastolic diameter (LVD), and left ventricular end-systolic diameter (LVS) were increased after chemotherapy, while left ventricular ejection fraction (LVEF) value was decreased. Before and after chemotherapy, the differences between BMI [(22.30±1.88) kg/m 2 vs. (23.59±2.32) kg/m 2] and LVEF [(63.69±4.69)% vs. (59.08±4.28)%] were statistically significant ( t = 3.40 and 4.98, all P < 0.05). The range of ΔBMI was 0-41.3%, and the range of the change of LVEF (ΔLVEF) was 0-15.9%. There was a significant correlation between ΔLVEF and ΔBMI ( r = 0.709, P < 0.001). The incidence of cardiotoxicity was 21.2% (36/170). Logistic regression analysis showed that BMI( OR = 1.639, 95% CI 1.263-2.127, P = 0.000) and ΔBMI ( OR = 1.147, 95% CI 1.071-1.228, P = 0.000) were independent risk factors for cardiotoxicity in early breast cancer patients undergoing anthracycline chemotherapy. According to the cardiotoxicity, the area under the ROC curve of ΔBMI and BMI was 0.757 and 0.687, respectively. When the ΔBMI value was 4.28%, the maximum Youden index was 0.399, the sensitivity was 0.750, and the specificity was 0.649. Conclusion:For breast cancer patients treated with anthracycline chemotherapy, ΔBMI can be used as an effective indicator for predicting cardiotoxicity; when ΔBMI exceeds 4.28%, the risk of cardiotoxicity is high.

Objective@#To investigate the predictive effect of interleukin-27clinical pathological factors on local tumor progression after microwave ablation of liver cancer.@*Methods@#Collection of blood samples from 102 patients with hepatic malignancies treated with microwave ablation from January 2014 to December 2016 in Department of General Surgery, Lianyungang Oriental Hospital of Xuzhou Medical University. There were 80 males and 22 females, aged 24 to 83 years old, mean (58.7±11.4) years old. Patient age, gender, hepatitis, tumor diameter, tumor number, tumor boundary, tumor type, tumor blood perfusion degree, liver Child-Pugh classification, whether liver cirrhosis and peripheral blood IL-27 concentration and tumor local progression relationship were analyzed.Detection of IL-27 concentration in peripheral blood using ELISA.The Youden index was calculated using the receiver operating characteristic curve (ROC) to obtain the best cutoff value for IL-27 levels.Patients were divided into 61 patients with high IL-27 (>498.45 pg/L) and 41 patients with low IL-27 (≤ 498.45 pg/L) with optimal cut-off value.Measurement data were expressed as mean ± standard deviation (Mean±SD), and comparison between groups was evaluated by t test; Comparison of count data was evaluated by chi-square test. Survival curves were plotted by Kaplan-Meier method and survival rates were calculated; Cox proportional hazard regression model was used for multivariate analysis.@*Results@#The total incidence of local tumor progression was 35.3% (36/102). The IL-27 value of 102 patients ranged from(34.92-1 014.87) pg/L.The ROC curve of IL-27 value was plotted based on 2-year tumor-free survival as the endpoint. The area under the curve was AUC of 0.714 (P=0.017). When the IL-27 value was 498.45 pg/L, the Youden index was the largest, the sensitivity was 0.682, and the specificity was 0.556.The 2-year disease-free survival rate of all patients was 64.2%.The 2-year disease-free survival rates of patients with high IL-27 and low IL-27 were 72.7% and 50.5%, respectively. Survival analysis by Kaplan-Meier method showed that the prognosis of patients with high IL-27 group was better than that of patients with low IL-27 group (χ2=5.991, P=0.014). Univariate analysis of tumor local progression was associated with tumor >3 cm (χ2=7.170, P=0.007), tumor adjacent to large vessels (χ2=9.301, P=0.002), and high degree of tumor perfusion (χ2=5.599, P=0.018), IL-27 high level (χ2=5.460, P=0.019). Cox proportional hazard model analysis of tumor>3 cm, tumor adjacent to large blood vessels, low IL-27 level was an independent risk factor for tumor progression.@*Conclusions@#The level of IL-27, tumor ≥3 cm and tumor adjacent large blood vessels are predictor of local tumor progression after microwave ablation of liver cancer. Patients with low IL-27 levels have a high rate of local tumor progression.

ABSTRACT

Objective To evaluate the effect of pulsed radiofrequency (PRF) on spinal adenosine triphosphate (ATP)-P2X4-NLRP3 signaling pathway in rats with neuropathic pain.Methods Forty healthy clean-grade adult male Sprague-Dawley rats,aged 2-3 months,weighing 220-260 g,were divided into 4 groups (n =10 each) using a random number table method:sham operation group (group S),neuropathic pain group (group NP),sham PRF group (group SPRF) and PRF group.Neuropathic pain was induced by chronic constriction injury to the left sciatic nerve of anesthetized rats.Rats received PRF treatment on 7th day after establishing the model in group PRF.The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured before establishing the model (T0) and at 3,7,10,14,21 and 28 days after establishing the model (T1-6).The rats were then sacrificed and the spinal cord was removed for determination of P2X4 and NLRP3 expression (by Western blot) and interleukin-1beta (IL-1β),IL-2,IL-6 and tumor necrosis factor-alpha (TNF-α) contents (by enzymelinked immunosorbent assay).Results Compared with group S,the MWT and TWL were significantly decreased at T1-6,the expression of P2X4 and NLRP3 was up-regulated,and the contents of IL-1β,IL-2,IL-6 and TNF-α were increased in NP,SPRF and PRF groups (P<0.05).Compared with group NP and group SPRF,the MWT and MWT were significantly increased at T3-6,the expression of P2X4 and NLRP3 was down-regulated,and the contents of IL-1 β,IL-2,IL-6 and TNF-α were decreased in group PRF (P<0.05).Conclusion The mechanism by which PRF alleviates neuropathic pain is related to inhibiting ATP-P2X4-NLRP3 signaling pathway in rats.

Objective@#To observe the short-term clinical efficacy of wrist arthroscopy in the treatment of Palmer IB type triangular fibrocartilage complex(TFCC) injury.@*Methods@#From March 2016 to January 2017, 12 patients with Palmer IB TFCC were admitted to the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine, including 5 males and 7 females, with an average age of 38.5 years.After examination and imaging examination, the diagnosis was confirmed.After full preoperative preparation, the patients underwent arthroscopic repair under wrist arthroscopy.The VAS score and modified Mayo wrist joint function score before and after surgery were tested to evaluate the clinical efficacy after 6 months.@*Results@#Twelve patients underwent successful operation and with no postoperative complications.The patients were followed up.The VAS scores before and after surgery were (4.08±0.90)points and (1.33±0.88)points, respectively, the difference was statistically significant(t=8.37, P=0.000). The modified Mayo wrist joint function scores were (48.33±7.48)points and (87.91±5.41)points before and after surgery, and the difference was statistically significant(t=12.05, P=0.000).@*Conclusion@#Arthroscopic repair of Palmer IB type TFCC has less trauma and quick recovery, and the clinical short-term result is satisfactory.

ABSTRACT

Objective To discuss the clinical characteristics for lateral cervical lymph node metastasis in stage cN0 papillary thyroid microcarcinoma and significance and feasibility of preventive dissection,and provide reference for clinical treatment.Methods Reviewd the clinical data of 191 patients with stage cN0 papillary thyroid microcarcinoma patients from Jul.2011 to Dec.2016 underwent surgery in the Department of General Surgery of Lianyungang Oriental Hospital.Assessed the need for preventive cervical lymph node dissection.Chisquare test and logistic regression were used to analyze the relationship between cervical lymph node metastasis and gender,age,tumor number,tumor size,capsule infiltration,single and bilateral tumors,Hashimoto's disease,and central lymph node metastasis.Results The positive rate of cervical lymph node metastasis in papillary thyroid microcarcinoma was 27.9% (50/191).Univariate analysis showed that the metastasis of the cervical lymph nodes was associated with infiltration of the capsule,Hashimoto disease,and CLN metastasis (all P < 0.05).Multivariate logistic regression analysis showed that the capsule infiltration (OR =7.563,P =0.000),Hashimoto's disease (OR =4.635,P =0.003),and central lymph node metastasis (OR =3.075,P < 0.001) were able to be independent risk factors for cervical lymph node metastasis.When the positive number of lymph node metastasis in the central region was ≥ 2,the positive rate of cervical lymph nodes was significantly increased (P < 0.001).Eleven patients (5.8%) had temporary recurrent laryngeal nerve palsy,29 patients (15.1%) had transient hypoparathyroidism,and no patients with permanent recurrent laryngeal nerve palsy and hypoparathyroidism.Conclusions The removal of the cervical lymph nodes helps to accurately classify the tumor and assess the risk.It is important to choose the postoperative treatment follow-up plan for patients.For patients with capsule infiltration,Hashimoto's disease,and central lymph node metastasis,cervical lymph node dissection should be routinely performed.

ABSTRACT

This study was aimed to evaluate the clinical effect and safety of Qing-Fei Tong-Luo (QFTL) ointment for treating children with pneumonia.Randomized controlled trial (RCT) was conducted among 460 cases of children with pneumonia.The observation group was given QFTL ointment combined with basic treatment.And the control group was only treated by basic treatment.Evaluation was given on the total clinical efficacy,disappeared time of fever,cough,expectoration,shortness of breath,and medication safety.The incidence of respiratory diseases was followed up on the 30th days after drug withdrawal.The results showed that in the aspect of clinical efficacy between two groups,the cure rate of the observation group was 98.26%,and that of the control group was 93.89%,with statistic significance (P < 0.05).The cure rate of the observation group was better than that of the control group.There was statistical difference on expectoration disappeared time (P < 0.05).There was no statistical difference on disappeared time of fever,cough and shortness of breath (P > 0.05).There was statistical difference on the incidence of respiratory diseases on the 30th days followed-up after drug withdrawal (P < 0.05).There was no statistical difference on the incidence of upper respiratory tract infection,pneumonia and asthma (P > 0.05).No adverse reactions occurred in the observation group.It was concluded that QFTL ointment combined with basic therapy on the treatment of pneumonia in children was significantly better than the control group in the aspect of clinical efficacy,expectoration disappeared time and the incidence of bronchitis.It is safe and effective.The prognosis is good and worthy of promotion in the clinical practice.

ABSTRACT

Objective To study the relationship between fucoidans structure and their antioxidant activities. Method Fucoidans (HF) were extracted from Laminaria japonica and was separated by anion exchange chromatography on DEAE-Cellulose 52 column (2.5?40 cm), and two fractions HF1 and HF2 were obtained. Fucoidans of low molecular weight (LF) was made after hydrolysis of HF by hydrochloric acid and purification by ultrafiltration. Chemical methods and gas chromatography were used to analyse the sugar composition of these polysaccharides; and infrared spectrum was utilized to determine their structure. The activity of inhibiting hydroxyl radical (?O H ) was evaluated by means of Fenton reaction. Results In HF1, 18.834% uronic acid, 44.197% L-fucose, 31.193% D-galactose and 24.612% D-mannose were detected. As to HF2, 5.878% uronic acid, 81.119% L-fucose and 18.881% D-mannose were found. L-fucose content in LF was 13.033%, equal to 32.084% of that in HF 40.621%. Both in HF1 and LF, sulfate groups were at the C-2 and C-4 positions of L-fucose, while in HF2, sulfate groups only at C-4 position. The concentration of fucoidans inhibiting 50% ?O H (IC50) was 7.6, 5.2 and 8.9 mg/ml for HF, HF1 and HF2 respectively. Nevertheless, LF approximately lost this activity. Conclusion The molecular weight, content and composition of sugar and the combining sites of sulfate groups were found to be related with the activity of inhibiting ?O H radical in fucoidans.
